*If you are a new client and have scheduled a massage, please download a health history form to print and bring in with you. Simply right click on Intake Form, choose "save target as", and when saved to your hard drive you can print it from there. It is in doc format so most programs will be able to open it. Please make sure you hand print in the responses in blue or black ink.
